Hospice & Palliaitive Care of Northeastern Illinois Names New Team Medical Director

Tracey Hashemi, M.D., was recently named Blue Team Medical Director of Hospice & Palliative Care of Northeastern Illinois (HPNI).
The agency’s “Blue Team” predominantly serves patients who reside in portions of Lake, McHenry and northern Kane counties, and is one of four team territories covered in the its six-county service area.
Dr. Hashemi currently serves as Director of Rehabilitation and Extended Care Services with Hashemi Medical Group in South Barrington. A board certified emergency physician, she is an attending physician at the Northwest Community Hospital Department of Emergency Medicine.
Having spent many years as an emergency physician at Little Company of Mary Hospital in Evergreen Park, Holy Cross Hospital and St. Elizabeth’s Hospital in Chicago, Dr. Hashemi recently served as medical director for Family Hospice.
“I became interested in HPNI because it is the 'Cadillac' of hospices in this area,” Dr. Hashemi said. “HPNI has a great reputation and I am eager to be part of its organization. My goal as medical director is to help provide excellent medical care as well as emotional and psychological support to our patients.”
