LIFT (Life Improves with Friends in Transition)

What is LIFT?

LIFT - stands for Life Improves with Friends in Transition - a predominantly volunteer-based free program for those diagnosed with a life-limiting disease, yet not ready for traditional hospice care.

What’s the cost?

NOTHING! There is no charge to participate in the LIFT program. The program is supported through donations and grants.

How do I enroll?

You may enroll in LIFT by calling Hospice & Palliative Care of Northeastern Illinois to set up an appointment with our program coordinator. She will speak with you and your family to determine your needs, and notify your physician should you participate in the program.

Who can participate?

Clients participating in LIFT have been diagnosed with a terminal illness and may decide to:

• Continue aggressive treatment (i.e. chemotherapy, dialysis, etc.)
• Not participate in traditional hospice care Benefit from basic case management and the support of a specially trained volunteer.

What will I receive?

Participants in the LIFT program receive a number of non-clinical services. These services include:

• A Care Partner/Consultant who will assess your short- and long-term care needs and provide recommendations and referrals for you to access appropriate medical, rehabilitation and support services. An Individualized Care Plan that will address basic needs and the issues most important to you. This plan is especially helpful when you need to access multiple services from multiple providers.
• Assistance in getting connected with community resources appropriate for your needs (i.e. care-giving agencies, lending closets, etc.). Your own professionally trained volunteer who will provide up to four hours a week of service. All volunteers are professionally trained to provide several kinds of support including:
  • Companionship
  • Respite breaks for family and caregivers
  • General errands
  • Transportation
  • Food preparation or meals
  • Light housekeeping
