Hospice Services
Levels Of Care
Hospice & Palliative Care of Northeastern Illinois provides four levels of care to meet the needs of patients and their families.
These levels include:
Routine Care
Care provided in the comfort and security of the patient’s home, wherever that is – a private residence, assisted living facility, or skilled nursing facility. A hospice nurse will work with a patient’s primary physician to bring the appropriate hospice services to you. Members of your hospice team will educate caregivers to help them feel comfortable performing routine care activities.
Respite Care
Short term care, to support the family or caregivers while continuing to meet the patient’s care needs. Care is usually provided in a skilled nursing facility.
General Inpatient Care
Care provided for a short-term stay in one of HPNI’s inpatient units in Barrington or Woodstock, or hospital when 24-hour skilled nursing care is necessary to help manage symptoms that require constant intervention.
Continuous Care
Care provided for short periods in the home (i.e., private residence, assisted living community, or long term-care community) to provide acute level of care for pain or symptom crisis.
