Education

Pain Management

Patients with serious or chronic illnesses may benefit from a disease specific care program to help manage pain and/or symptoms. Our palliative care team will create a specific plan that encompasses the whole person. Patients who may benefit from palliative care include anyone who has experienced uncontrolled symptoms related but not limited to:

Congestive Heart Failure
Kidney Illness
Stroke

COPD
Dementia/Alzheimer’s
