LPN - 8/25/2010
The LPN provides direct care under the supervision of a Hospice Registered Nurse. The LPN is responsible for following the plan of care, providing treatment and working collaboratively with the members of the team in order to ensure comfort and quality of life.
We are looking for a full time LPN to work in Cook, DuPage, and Kane counties, working with patients in their home and/or other facilities from 12pm-8:30pm Monday – Friday and rotating every 5th or 6th weekend.

The CNA is responsibile for providing personal care and homemaking services to create and maintain a safe and comfortable environment. The CNA understands the use and care of supplies and equipment related to personal care, and is able to identify and report problems related to patient care.
We a full time CNA position that will work flexible hours in our southern territory (Cook, Kane and DuPage Counties) working with patients and families in assisted living facilities, this position includes rotating weekends.
We are also looking for CNAs to work on a PRN/as needed basis to fill in when staff are on vacations and when our census is high.

We are looking for a part time Clinical Systems Educator (working 20 hours per week). They are responsible for responsible for clinical systems education, clinical documentation upgrades, and participating in clinical process improvements.
Clinical Team Manager - 8/25/2010
We are looking for a full time Clinical Team Manager. They are responsible for coordinating and managing the care of patients and families. They verify insurance coverage and appropriateness of medications and medical equipment for each patient. They manage financials associated with the team’s patients. The team manager also manages the RNs, CNAs, and LPNs on their team.
Patient Care Services Assistant - 8/25/2010
The Patienc Care Services Assistant assists with clerical functions for the department such as making copies, retrieving records from Health Information, printing visit notes for LTC charts and faxing physician orders. They will work 20 hours per week.
Social Woker - 8/11/2010
Responsible for providing assessment of the needs of Hospice of Northeastern Illinois patients and families. They also provide the patients and families with emotional and resources support. The social worker provides counseling regarding the dying process.

Accounts Payable - 8/5/2010
The Accounting Specialist/Accounts Payable processes incoming invoices for patients and vendors in accordance with company accounting control policies and procedures. They also support the financial closing process. The Accounting Specialist/Accounts Payable ensures timely payment of invoices. This position is full time and will be scheduled to work 40 hours per week (Monday-Friday 8:30am-5pm)
Helpdesk Specialist - 7/23/2010
The Helpdesk Specialist is the point of contact for help related to technology including; computers, phones, internet, and others. They are responsible for updating and maintaining current technology and software used by Hospice of Northeastern Illinois. The Helpdesk Specialist also provides training for use of equipment and software.
-This position will work 20 hours per week.
