Helpdesk Specialist - 7/23/2010

The Helpdesk Specialist is the point of contact for help related to technology including; computers, phones, internet, and others. They are responsible for updating and maintaining current technology and software used by Hospice of Northeastern Illinois. The Helpdesk Specialist also provides training for use of equipment and software. -This position will work 20 hours per week.


RN Patient Care Liaison - 6/15/2010

Responsible for effectively meeting the needs of patients/families primarily in the acute care hospital setting. Will respond to the hospital based referral sources to ensure a smooth transition, for the patient, from the acute care setting to Hospice care. Will provide Hospice and Palliative Care education to the facility staffs to ensure continuity of care for the patient as needed. We have a position that will be working Monday-Friday 10am-6:30 pm with rotating weekends


Intake / Care Coordinator RN - 5/25/2010

The Intake / Care Coordinator will work various shifts and is the first contact with families, doctors and health care facilities inquiring about Hospice and Palliative Care of Northeastern Illinois. It is fast paced and if you enjoy customer contact followed with detail and follow through; this is for you! Responsible for obtaining information relating to the admission of a patient and obtaining verbal authorization of acceptance by the patient’s doctor. The Intake / Care Coordinator is responsible for processing this information into the computer for clinical staff to view and follow up with. QUALIFICATIONS: Training in hospice and palliative care and pain and symptom management Training in patient care coordination RN or LPN licensed in the state of Illinois 3-7 years of experience in patient care coordination. Case management skills required Demonstrated commitment to hospice philosophy of care. Has available means of transportation that allows for reliable visits. Detailed oriented Organization skills Familiarity and comfort with computer documentation, a must. (we use electronic medical record), Able to multi-task and work with frequent interruptions. Spanish speaking, a plus. EDUCATION: RN or LPN currently licensed in the state of Illinois. BSN preferred.


RN / Registered Nurse Nights and Weekends - 3/4/2010

Responsibilities include assessment, planning and interventions to maximize the comfort and health of patients and families. We have part time and full time positions available during the week in the extended hours (Monday-Friday 5pm-8:30am and Saturday and Sunday) in our 6 counties in Illinois; Cook, Lake, DuPage, Kane, Boone and McHenry, and in our inpatient unit in Woodstock. We have a position that will work every Friday and Sunday 5pm-8:30am and every other Saturday 5pm-8:30am in our 6 counties in Illinois; Cook, Lake, DuPage, Kane, Boone and McHenry, and in our inpatient unit in Woodstock. We have one position that will work every Saturday and Sunday 20 hours per week


RN / Registered Nurse - 12/21/2009

Responsibilities include assessment, planning and interventions to maximize the comfort and health of patients and families. We have one part time position that will work every Saturday and Sunday 8 hours per day, working with patients in their homes and/or in facilities located throughout our service area(Cook, Lake, DuPage, Kane, Boone and McHenry). We have one full time position that will work every Friday, Saturday and Sunday 8pm-8:30am, working with patients in their homes and/or in facilities located throughout our service area (Cook, Lake, DuPage, Kane, Boone and McHenry). We are looking for a full time RN to work in our Southern counties (DuPage and Kane), working with patients in their home and/or other facilities. We have Resource RN positions (working on an as needed basis) available, working with patients in their homes and/or in facilities located throughout our service area (Cook, Lake, DuPage, Kane, Boone and McHenry). We are also beginning to recruit for part time and full time RNs to work 12 hour shifts at our new Pepper Family Hospice Home and Center for Care.


LPN - 12/21/2009

The LPN provides direct care under the supervision of a Hospice Registered Nurse. The LPN is responsible for following the plan of care, providing treatment and working collaboratively with the members of the team in order to ensure comfort and quality of life. We have Resource LPN positions (working on an as needed basis), working with patients in their homes and/or in facilities located throughout our service area (Cook, Lake, DuPage, Kane, Boone and McHenry), this position includes rotating weekends. We are also beginning to recruit for part time and full time LPNs to work 12 hour shifts at our new Pepper Family Hospice Home and Center for Care.


CNA - 12/21/2009

The CNA is responsibile for providing personal care and homemaking services to create and maintain a safe and comfortable environment. The CNA understands the use and care of supplies and equipment related to personal care, and is able to identify and report problems related to patient care. We have an Night CNA position that will work from 12am-8:30am 5 days per week, working with patients in their homes and/or in facilities located throughout our service area(Cook, Lake, DuPage, Kane, Boone and McHenry), this position includes rotating weekends. We are also looking for CNAs to work on a PRN/as needed basis to fill in when staff are on vacations and when our census is high. We are also beginning to recruit for part time and full time CNAs to work 12 hour shifts at our new Pepper Family Hospice Home and Center for Care.
