-
- Routine Care Care provided in the comfort and security of your home, wherever you call home – private residence, assisted living facility, or skilled nursing facility. A hospice nurse will work with a patient’s primary physician to bring the appropriate hospice services to you. Members of your hospice team will educate caregivers to help them feel comfortable performing routine care activities.
- Respite Care Short term care, to support your family or caregivers while continuing to meet your care needs. Care is provided in a skilled nursing facility short term.
- General Inpatient Care Care provided for a short-term stay in one of HPNI’s inpatient units in Barrington or Woodstock, when 24-hour skilled nursing care is necessary to help manage symptoms that require constant intervention.
- Continuous Care Care provided for short periods in your home (i.e., private residence, assisted living community, or long term-care community) to provide acute level of care for pain or symptom crisis.
- Grief Support: Our Center for Care offers one of the largest and most comprehensive hospice grief support programs in the State of Illinois, including individual and group counseling, workshops and ongoing support. Our services are offered to families of our hospice patients as well as to the general public.
- Accessible: We are here 24 hours a day, seven days a week, 365 days a year. Just call 224-770-2489.
- Not-for-profit: Not-for-profit does not mean unsuccessful. It means we are not focused on profits, but rather on delivering quality care and support to all hospice and palliative care patients who are referred to us, regardless of their ability to pay.
- Palliative Care: If a loved one has a serious illness and needs relief from pain or uncontrolled symptoms, our palliative team can help. Palliative care is appropriate for people of any age and at any point in an illness, and can be delivered along with curative treatments.
